Coverage is provided by Guarantee Trust Life Insurance Company and administered by Student Athletic Protection, Inc.
Student Athletic Protection Insurance will process medical bills resulting from an accident sustained in the course of a scheduled game or supervised school practice (this excludes any practice that your son or daughter does on his/her own time).
A $50 deductible is applied to each incident. The deductible is applied to the balance of bill, after the primary insurance has paid.
If you need to file a Student Athletic Protection Insurance Claim Form:
1. Notify your coach, trainer, and/or athletic director/coordinator of injury immediately. The coach or trainer will complete a SBCSC Accident Report and file it with the Athletic Director/Coordinator's Office.
2. If medical treatment results from the injury (trip to the doctor, emergency room, etc.), please file with your insurance carrier first. Never list the school as your insurance carrier or the responsible party, as this delays the processing of claims and timely payments.
3. Obtain the partially completed Student Athletic Protection Claim Form from your Athletic Director/Coordinator by scheduling an appointment. They will complete and sign the lower half of the form.
4. Complete the upper half of the form, sign and include statement of benefits (paid or denied) from your primary insurance carrier. Each charge must be processed by your primary carrier before it can be processed by Student Athletic Protection, Inc.
5. Make copies of the form and any Explanations of Benefits/bills for your files.
6. Mail the claim form and original bills within 90 days to:
